Sunday, August 16th, 11 am — 1 pm Morgan’s Beach, Route 155, Enfield. Join the G.E.A.R. Parent Network at Morgan’s Beach on the beautiful western shore of Cold Stream Pond for a relaxing day of family fun, community, and connection. This is a wonderful opportunity to spend quality time together while meeting other parents and caregivers in a welcoming, supportive environment. G.E.A.R. Parent Network will cover the beach entrance fee at no cost for participating families. Families are welcome to stay and enjoy the beach for the rest of the day. Reserve your spot here.

Evidence demonstrates that integrating care for populations with co-occurring conditions can improve health outcomes. This report describes the evolving landscape of integrated care in the United States, including established frameworks that emphasize interdisciplinary teams, clear communication systems, and care coordination across providers.
